Create a Survey Using Forms

1. Brainstorm for your survey in a Google Doc. Your brainstorming document should include a hypothesis for your survey. Check out possible Topics and Hypotheses for your investigation. Also, check out the graphs and data collect from this professional Technology Survey.

2. View the Requirements for your Survey (Form)

3. Check out the following sample questions and discuss if they are good or bad questions for a survey.

4. Open up Google Apps and log in.

5. Create your Form.

A.Click on DOCS.

B. Go to NEW and choose FORMS

C. Create your Survey using the Form. Don't forget to check the Requirements for your Survey (Form)

D. Check the "Require Sign In" box at the top of the survey when you are in Edit mode.

E. In the large box under your Survey Title, you must type a statement that explains the purpose of your survey. "The purpose of this survey is to..."
